Responsibilities:
* Conduct comprehensive assessments of adults in need of social care support.
* Develop and implement person-centered care plans in collaboration with service users, their families, and other professionals.
* Provide advocacy and support to ensure service users’ rights and preferences are respected.
* Work collaboratively with multi-disciplinary teams to coordinate and deliver high-quality care and support services.
* Monitor and review care plans regularly to ensure effectiveness and adaptability to changing needs.
* Safeguard vulnerable adults and respond appropriately to safeguarding concerns.
* Maintain accurate and up-to-date records in accordance with statutory requirements and organizational policies.
Requirements:
* Qualified Social Worker with current registration with Social Work England.
* Proven experience working as a social worker with adults, preferably within a statutory setting.
* Sound knowledge of relevant legislation, policies, and procedures related to adult social care.
* Strong assessment, analytical, and decision-making skills.
* Excellent communication and interpersonal skills, with the ability to engage effectively with diverse individuals and groups.
* Ability to work independently and as part of a team in a fast-paced environment.
* Commitment to continuous professional development and reflective practice.
