Job Description:
Social Worker, £26ph, LAC, Durham
Pertemps are recruiting for qualified and experienced Social Workers to join the Children in Care service and work with 16- and 17-year-olds to support them through to adulthood. This will involve closely working with the Care Leaver service and Home Finder service to ensure robust plans are in place. It will involve updating Pathway Needs Assessments and Pathway Plans as well as referring to the Navigation Team early on should young people need adult social work intervention once they turn 18 years old. The postholder will be expected to visit children in accordance with the visiting frequency, hold monthly care team meetings as well as attending PEP meetings and Children in Care reviews.
The successful candidate will need to have experience of working with children looked after and experience with transitional working to support children into adulthood.
There is plenty of hybrid working available with the expectation to work in the office at least two days a week.
