Job Description:
Post Title: Personal Adviser
Service Area: Stockport Family
Type: Full time, temporary for 3 months with possible extension
Job Purpose:

To work with young people aged 16-25 years who are or have been looked after or accommodated by the Local Authority, in order to plan their transition to adulthood and assist them to live an independent life in their community.
Responsibilities:
* Community Resources:

Have a working knowledge of community resources available to parents and carers and introduce them as appropriate.

* Support Services:

Promote the Care Leaver Forum, mentoring scheme, and other support services for young people.

* Advice and Guidance:

Provide advice, guidance, and support to social workers preparing looked after children aged 16+ for transition to adulthood. Participate in delivering briefings and training as required.

* Statutory Responsibilities:

Carry out statutory responsibilities and duties for looked after children in line with the Children Act 1989, Children (Leaving Care) Act 2000, and related regulations.

* Case Management:

Manage a caseload, keep accurate case records of work with young people, and undertake statutory recordings.

* Pathway Planning:

Contribute to Pathway Plan Assessment and provide ongoing assessment with young people to inform Pathway Planning.

* Assessments:

Undertake assessments of young people resuming or wishing to resume education/training or presenting with other support needs post 21 years.

* Welfare Promotion:

Work with young people to promote welfare and ensure their views and wishes are taken into account when undertaking assessments, developing Pathway Plans, and reviews.

* Practical Support:

Provide practical advice, assistance, and support to young people.

Requirements:
* Experience of working with care leavers or relevant experience working with young people.
* NVQ Level 3 in childcare or other relevant professional qualification.
* Ability to drive and access to a car for work.
* Understanding of a range of issues related to care leavers/young people.
* Fluency Duty: The ability to converse at ease with service users/customers and provide advice in accurate spoken English (to be included when the role is covered by the fluency duty).
