Key Responsibilities:
* Conduct comprehensive assessments of new clients to identify their care needs and develop tailored care plans.
* Review and update existing care plans to ensure they meet client’s evolving needs
* Coordinate and schedule care services for clients, ensuring optimal coverage and timely delivery of care.
* Communicate effectively with care staff to allocate tasks and ensure clarity in service delivery.
* Provide support and guidance to care staff, fostering a collaborative and supportive working environment.
* Conduct regular team meetings and training sessions to address any issues and ensure high standards of care.
* Act as the primary point of contact for clients and their families, addressing any concerns or queries promptly and professionally.
* Foster strong relationships with clients and their families to ensure satisfaction with our services.
* Ensure all care activities comply with relevant regulations and best practice guidelines.
* Maintain accurate and up-to-date records of client care plans, assessments, and service delivery.
* Respond to emergencies and unforeseen circumstances effectively, ensuring the safety and well-being of clients and staff.
Profile
Requirements:
* Previous experience in a care coordination or similar role within the health and social care sector.
* Strong understanding of care planning, assessment, and delivery processes.
* Excellent organisational and time-management skills.
* Compassionate and empathetic nature, with a genuine commitment to improving the lives of clients.
* Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and care staff.
* Knowledge of relevant regulations and best practice guidelines in the care sector.
* Proficiency in using care management software and Microsoft Office applications.
