Responsibilities Include:
* Care Act assessments
* Discharging statutory duties under the Care Act 2014
* Support with waiting list backlog of first assessments
* Working as part of a multi disciplinary team
* Assessments and Unscheduled Reviews
* General case management including care planning and safeguarding
* Action safeguarding procedures



Requirements:
* At least 2 years of relevant experience post qualification
* Social Work England registration
* Social Work Qualification
