Responsibilities Include:
* Attending Multi-Disciplinary meetings and completing initial Safeguarding enquiries.
* Responsible for Care Planning and providing short term case management safeguarding enquiries.
* Assessments and Unscheduled Reviews
* Undertaking assessments under the Care Act 2014 and the Mental Capacity Act 2005
* Action safeguarding procedures



Requirements:
* At least 2 years of relevant experience post qualification
* Social Work Qualification and SWE registration
