Pay Rate: £22 per hour
Pay Mechanism: Direct Engagement
Contract Duration: 14th October 2024 - 14th January 2025

Job Overview:
We are seeking a dedicated and experienced Band 6 Occupational Therapist to join our community healthcare team at Norfolk Community Health and Care NHS Trust. The successful candidate will provide high-quality care, supporting patients in the community to promote their independence and well-being. You will manage a diverse caseload, working across a range of community settings, providing assessments, developing care plans, and delivering interventions to enhance the daily living skills of your patients.
Key Responsibilities:
*Conduct comprehensive assessments of patients in their homes and community settings.
*Develop and implement individualized treatment plans.
*Collaborate with multidisciplinary teams to ensure holistic care.
*Support patients in regaining their independence and improving their quality of life.
*Manage a varied and complex caseload with autonomy.
Key Requirements:
*Degree in Occupational Therapy.
*HCPC registration.
*Experience in community or similar settings.
*Excellent communication and team-working skills.
