Key Responsibilities:
*Undertake comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.
*Develop and implement person-centered care plans that promote independence and well-being.
*Provide support and guidance to individuals and their families, addressing complex challenges and promoting positive outcomes.
*Collaborate with multidisciplinary teams, including healthcare professionals, housing providers, and other relevant agencies.
*Advocate for the rights and needs of individuals with learning disabilities, ensuring they have access to appropriate services and support.
*Maintain accurate and up-to-date case records, adhering to professional standards and legal requirements.
Essential Requirements:
*Social Work qualification (Degree or Diploma level)
*Registration with Social Work England
*Proven experience working with individuals with learning disabilities
*Strong assessment and care planning skills
*Excellent communication and interpersonal skills
*Ability to work independently and as part of a team
*Commitment to promoting person-centered practice
*Flexibility to work outside of traditional office hours as required
