Job Description:
Job Overview:
We are currently recruiting for a dedicated and skilled Occupational Therapist to join an Adult Social Care team within a well-established local authority. The successful candidate will play a vital role in delivering high-quality care and support to adults and their carers, ensuring the best possible outcomes through holistic, person-centred assessments and interventions. This is a fantastic opportunity to contribute to a dynamic, multi-disciplinary team focused on improving the lives of vulnerable adults in the community.
Key Responsibilities:
*Carry out comprehensive assessments using a strengths-based, person-centred approach, focusing on the needs and assets of individuals.
*Develop and implement care plans and interventions to support independence, wellbeing, and quality of life for people and their carers.
*Manage a caseload, including complex cases, ensuring appropriate risk assessments and safeguarding procedures are followed.
*Collaborate with health services and other partners to ensure people receive the right support and are discharged from hospital safely and promptly.
*Actively contribute to safeguarding adults and promoting independence through the use of personal budgets and community resources.
*Provide expert advice and support on rehabilitation plans, home adaptations, and equipment needs.
*Undertake statutory duties under the Care Act, Mental Capacity Act, and relevant safeguarding legislation.
*Engage in reflective practice and continuous professional development to maintain high standards of care and service delivery.
*Provide supervision and support to non-qualified staff and students, where applicable.
Qualifications & Experience:
* Essential:
*Professional Occupational Therapy qualification (HCPC registered).
*Experience in completing assessments, care planning, and reviews in an Adult Social Care or Health setting.
*Experience of safeguarding, mental capacity assessments, and best interest decision-making.
*Knowledge of relevant legislation including the Care Act, Mental Capacity Act, and Safeguarding Adults policies.
*Strong communication skills with the ability to engage effectively with service users, carers, and multi-disciplinary teams.

* Desirable:
*Experience working in a community or hospital setting.
*Ability to mentor and supervise less experienced staff or students.
*Experience with Personal Budgets, Disabled Facilities Grants, or similar community-based resources.
