Location: Leeds Community Healthcare NHS Trust
Pay Rate: £22 per hour
Pay Mechanism: Direct Engagement
Contract Duration: 25/11/2024 - 25/02/2025

Job Overview We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at Leeds Community Healthcare. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.
Key Responsibilities *Assess, plan, and deliver occupational therapy interventions for patients in the community.
*Work with multi-disciplinary teams to support the rehabilitation and recovery of patients.
*Develop and implement tailored care plans to meet individual needs.
*Provide advice and education to patients, families, and carers.
*Manage a caseload, ensuring timely and effective interventions.
Key Requirements *HCPC registration as an Occupational Therapist.
*Proven experience in community-based occupational therapy.
*Strong communication and interpersonal skills.
*Ability to work independently and as part of a team.
*Experience in managing complex caseloads is desirable.
