About this job
We are looking for a Social Worker to join our Short Breaks Review Team in the Children & Young People with Disability Service. We pride ourselves on our relationship-based approach to social work and ensure that our children and young people are at the heart of all we do.
The Short Breaks Review Team sits within our Early Help provision and aims to support children with disabilities and complex needs, developing, implementing and reviewing holistic and multi-agency support packages to ensure children are safe and able to achieve to their full potential.
Your role will be to complete reviews of care packages, working closely with colleagues and multi-agency partners, and developing creative support plans which maximise independence and quality of life. You will listen to the views of families and carers and consider their needs alongside ensuring that the voice of the child is at the centre of all planning. You will present your recommendations to the Resource Panel for authorisation. You will be fully supported by your team manager.
Special requirements for this post
We welcome applications from qualified and experienced candidates. You will need to have a Social Work degree and have completed your ASYE. You will need to hold Social Work England registration.

About you
You will be an experienced social worker able to work in a busy environment and to prioritise your work to ensure key performance targets are met.
You will have a commitment to building relationships with children, young people, and their care givers and using this approach to develop effective packages of support. You will have an awareness of the particular challenges faced by children and young people with complex needs and disabilities.
The work involves liaising with partners both within and without the council as necessary to ensure that services are provided efficiently and effectively and always focus on the best outcome for the child or young person. You will be competent at writing high quality reports for presentation to senior managers, and skilled in advocating for the children and young people you are working with. You will be able to identify safeguarding concerns and refer on in a timely way
