Job Overview:
We are seeking an experienced Band 6 Occupational Therapist to join our community team, working in Marlow and Thame. This role focuses on elderly care, conducting community visits, and delivering person-centered therapy to enhance patients’ quality of life and independence.
Key Responsibilities:
*Manage a community-based caseload, focusing on elderly care and rehabilitation.
*Conduct home assessments and implement interventions to improve safety and independence.
*Collaborate with multidisciplinary teams to coordinate comprehensive care plans.
*Provide advice and support to patients, families, and carers to achieve therapy goals.
*Document patient progress and maintain accurate records in line with trust policies.
Key Requirements:
*HCPC registration as an Occupational Therapist.
*Proven NHS experience in community and elderly care settings.
*Knowledge of current practices in community rehabilitation and home safety adaptations.
*Strong communication, organizational, and teamwork skills.
*Access to a vehicle and a valid driving license for community visits.
