Job Overview We are seeking an experienced Band 6 Occupational Therapist to join the team at Manchester University NHS Foundation Trust. The role involves conducting specialist assessments and discharge planning within an acute medicine environment. This position offers the chance to make a meaningful impact on patient care while working as part of a supportive multidisciplinary team.
Key Responsibilities *Conduct detailed assessments to identify patients’ needs in an acute medical setting.
*Collaborate with patients, families, and multidisciplinary teams to plan and implement effective discharge strategies.
*Offer expert advice on rehabilitation and ongoing care plans to ensure continuity and quality of care.
*Maintain accurate and thorough documentation in line with Trust policies.
*Actively participate in case reviews, team meetings, and service improvement initiatives.
Requirements *HCPC registration as an Occupational Therapist.
*Demonstrated experience in acute medicine, particularly in assessment and discharge planning.
*Strong clinical reasoning and decision-making abilities.
*Excellent communication and teamwork skills.
*Dedication to delivering compassionate, patient-centered care.
