About Us:
Randstad is delighted to collaborate with the Adult Community Wellbeing Teams in North Tyneside,to recruit a compassionate and dedicated Social Worker.
This role offers the opportunity to work with adults in the community, supporting their independence, well-being, and access to essential services.
Job Description:
As a Social Worker in the Adult Community Wellbeing Team, you will manage a diverse caseload, working with individuals to assess their needs and provide tailored support.
You will play a crucial role in enabling adults to lead fulfilling lives while ensuring their safety and well-being.
Responsibilities:
*Conduct person-centred assessments to identify individuals’ needs, risks, and strengths.
*Develop and implement support plans that promote independence and enhance quality of life.
*Work collaboratively with health professionals, community organisations, and other agencies to deliver holistic care.
*Provide safeguarding interventions to protect vulnerable adults from harm or neglect.
*Support individuals to access appropriate services, including housing, health care, and financial assistance.
*Maintain accurate and up-to-date case records in line with statutory requirements and best practices.
*Regularly review support plans to adapt to changing needs or circumstances.
Qualifications:
*Qualified Social Worker registered with Social Work England.
*Experience working in adult social care or community wellbeing teams.
*Strong knowledge of relevant legislation, including the Care Act and safeguarding procedures.
*Excellent communication, assessment, and planning skills.
*Ability to work effectively as part of a multidisciplinary team and independently.
