Location: Central London Community Healthcare NHS Trust
Pay Rates: £23 per hour
Pay Mechanism: Direct Engagement
Working Hours/Days: 3-5 days per week (flexible; please indicate your preference)
Start Date: 15/11/2024 to 15/02/2025

Job Overview We are looking for a motivated and skilled Band 6 Occupational Therapist to join the community health team. This role involves providing holistic, patient-centered care in various community settings, supporting individuals with complex rehabilitation needs to achieve independence and improve their quality of life.
Key Responsibilities *Conduct assessments in home or community settings, developing personalized care and rehabilitation plans.
*Deliver high-quality interventions to enhance patients’ mobility, daily living skills, and mental well-being.
*Collaborate with multidisciplinary teams, including physiotherapists, nurses, and social care professionals, to provide comprehensive care.
*Educate patients, families, and caregivers on rehabilitation techniques, equipment use, and safety practices.
*Contribute to discharge planning, ensuring patients transition safely back to their home environment with proper support.
Requirements *HCPC registration as an Occupational Therapist.
*Experience in community healthcare or similar clinical settings.
*Strong assessment, planning, and rehabilitation skills with a patient-centered approach.
*Excellent communication and teamwork abilities, particularly in multi-agency collaboration.
*Ability to work independently and adapt to varied community environments.
