Job Role: Support Worker
Salary: £19,600 per year (full time equivalent £24,500)
Hours: 30 hours per week
Contract type: Permanent
Location: Basingstoke
Additional information:
*Valid driver’s license and access to a car for business purposes
*Participation in an Outreach rota which may include working early mornings, evenings and weekends
*Participation in an on-call, out-of-hours rota (£100 per week when on-call)
The Role:
In this role, you’ll be supporting individuals who have experienced homelessness and are now navigating the final steps of their journey toward stability. You’ll take the lead in managing referrals into the service, conducting assessments, and providing tailored, empowering support to help clients maintain their accommodation and achieve meaningful, positive changes in their lives.
Working across four units of accommodation located within two neighbouring properties, you’ll focus on fostering resilience, building essential skills, and guiding clients toward greater independence.
Responsibilities include:
*Manage referrals and assess clients, offering tailored support to help them maintain accommodation and work towards long-term independence
*Build effective relationships with partner agencies to deliver multi-agency support plans
*Keep accurate and timely records of all client interactions
*Empower a caseload of clients, to identify their strengths and goals, and develop their skills, confidence and well-being
