Job Description:
Pertemps Social Care are recruiting agency social work assistants for roles within a Hospital Team based in Manchester.
You will be working as part of a multi-disciplinary team, supporting professionals in assessing and facilitating safe and timely discharges with a commitment to promoting independence. You will work in a supportive team of Social workers and Social care support officers. The team works to tight deadlines, with a fast turnaround of work.
Your core responsibilities will include:
*Rapid Assessment under Discharge to Assess guidelines for hospital discharge
*Assessing service users, mental capacity and making best interest decisions
*Liaising with the discharge liaison team, as well as ward staff and family members.
*Arranging care packages on discharge and step down beds.
*Completing reviews and support plans.
*Reporting any delays with service users.
