*Experience in reablement services, equipment provision, and home adaptations.
*Strong knowledge of strength-based approaches and assessment techniques.
*Skills in manual handling and risk management.

Join our Promoting Independence Team as an Occupational Therapist, where you’ll play a vital role in delivering a therapy-led reablement service.
We work with individuals to assess their strengths and needs, providing equipment and adaptations to support them in reaching their personal goals. This is an opportunity to make a real difference in people’s lives, enabling them to regain confidence and live independently.
What You’ll Be Doing
* Conducting strength-based assessments to identify individual capabilities and support needs.
*Delivering reablement-focused interventions, helping individuals achieve their goals through tailored strategies.
*Assessing and recommending equipment and home adaptations to enhance safety and independence.
*Carrying out housing needs assessments, ensuring living environments meet long-term requirements.
*Providing expertise in moving and handling, including risk assessments and training for individuals and carers.
*Collaborating with multi-disciplinary teams to deliver person-centered care plans.
