Job Description:
We are looking for someone with a wealth of experience and commitment when working with our clients.
We offer various levels of support for Adults with learning disabilities/difficulties, Asperger’s and associated needs, Dementia, Alzheimer’s, and Huntingdon’s Disease, This may include communication difficulties, Autism, epilepsy, mental health and physical disabilities. We also work with adults with circumstances such as homelessness, drug/alcohol addictions, self harm and neglect.
Promoting Independence South West works and promotes strengths based and Person-Centred approaches; to mentor/support our clients to reach their potential and to live their lives to the fullest. We specialise in working with our clients, their support networks and other statutory, voluntary and charity sectors.
We offer diverse range of support. The team’s specialism will cover, cooking, budgeting, shopping, road awareness, accessing public transport, groups, activities out in the community, education, and any interests in seeking voluntary and/or paid work.
To ensure that our clients do not become stagnant throughout their journey of moving towards and/or maintaining their independence, monthly reviews will be undertaken by the Mentor Lead and the Mentor Co-ordinator - to identify strengths, achieved goals and/or consider alternative ways of support to assist our clients.
This post is not a carers or domiciliary role that ’do for people’ and manage personal care needs. Reablement is the key to the role of our Mentor Coordinators.
Our expectation is that as a member of our team you will be able to demonstrate excellent communication and mentoring skills, with the value base of respect, congruence and honesty and ultimately the acceptance of difference. What one person may desire when considering their independence may differ significantly from another, therefore, being able to respect this is paramount in our practice and work.
Our practice will include undertaking initial assessments of individual’s needs/requirements, and the ability to ascertain and balance risk, and how this can be managed when mentoring/supporting our customer’s is an required skill. Being able to work under one’s own initiative and to accurately record outcomes and daily plans are also essential.

There will be an initial two week intensive induction package, which will consist of undertaking mandatory training such as understanding legislations such as The Care Act 2014, The Mental Capacity Act 2005 and The Human Rights Act 1998, as well as understanding the Safeguarding Process, Health & Safety and bespoke training in Autism, Mental Health etc. Our service is learning environment; which promotes specialism whilst also reducing the risks of our practice becoming out of date and not promoting the independence for our customers.
Job description
We require a Mentor Coordinator to work with specific clients with individualised care and support plans generated from the Local Authority (Cornwall Council) and NHS.
We require cover within Bodmin, Par Newquay, St Austell, Wadebridge and surrounding areas (within a 10 mile radius) in Mid Cornwall.

About the role:
We offer flexibility of hoursavailable to suit your lifestyle (days, evenings and Saturdays). Our service is not open on Sundays.
You will make calls to service users throughout your shift and must be prepared to travel within the service catchment area.
Duties include
- Support with enabling our clients to access their local community - shopping, accessing public transport, undertaking meaningful activities out in the community, accessing voluntary/paid work
- Accompanying to medical appointments if needed;
- Support around managing complex behaviours, have an understanding of the perspective of the person and being aware of risk assessment/behavioural plans that are in place;
- Ability to complete daily case notes of the support undertaken - to detail the purpose of the support, and the outcome of the support provided;
- Ability to proactively work with partnering agencies, such as GP’s, Health workers, Local Authority staff, Mental Health workers, Advocates and family in a professional manner;
- Understanding of the principles of Person-Centred and Strengths based practice alongside legislation such as The Care Act 2014, The Mental Capacity Act 2005 and The Human Rights Act 1989 and how these inform your practice;
- Ability to lone work and have the initiative and confidence to review the concept and of risk and ensure that all practice is in the best interest of our clients;
- To actively participate in maintaining the service as a learning environment with your colleagues.

- Have an understanding and adhere to all service policy and procedures around confidentiality, administering of medication, health and safety, anti-discriminatory practice and the Safeguarding process and how it informs practice and decision making;
- Strong skills with mentoring others and performing in a professional manner; to maintain professional boundaries between yourself and our clients;

We require at least one year of experience of working vulnerable adults with diverse care and support needs. Knowledge and skills with working IT equipment and processes like Word, Outlook is mandatory.

A full driving licence and access to your own vehicle with ’business use’ car insurance is essential to undertake this role.
