Job Overview:
We are seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at NELFT. The successful candidate will work across diverse community settings, providing individualized care and empowering patients to achieve greater independence.
Key Responsibilities:
*Assess, plan, and deliver occupational therapy interventions for patients in the community.
*Collaborate with multidisciplinary teams to facilitate rehabilitation and recovery.
*Develop and implement tailored care plans to address patient needs.
*Provide advice and education to patients, families, and carers.
*Manage a caseload, ensuring timely and effective interventions.
Key Requirements:
*HCPC registration as an Occupational Therapist.
*Proven experience in community-based occupational therapy.
*Strong communication and interpersonal skills.
*Ability to work independently and collaboratively.
*Experience managing complex caseloads is desirable.
