Job Description:
Job Scope:
We are recruiting a Care Manager for our client, a reputable care organization dedicated to supporting individuals with complex needs. As a Care Manager, you will oversee a team providing supported accommodation to six female residents, aged 18-40, with mental health needs. Key responsibilities include managing daily care operations, scheduling shifts, providing supervision and on-the-job training, and ensuring person-centered care is delivered. You will also be responsible for conducting regular appraisals, managing staff performance, and fostering a positive, well-trained team.
As a Care Manager, you will lead risk assessments, ensure care plans are regularly reviewed, and coordinate multi-disciplinary meetings to support residents’ needs. You will be involved in managing referrals, overseeing medication administration, and ensuring compliance with regulatory standards. Additionally, you will take part in an on-call rotation every six weeks, responding to emergencies and ensuring high-quality care delivery.
Do you have?
*NVQ Level 2/3 in Health and Social Care.
*Proven experience managing care services within a community setting for individuals with complex needs.
*Strong leadership skills, with the ability to foster positive relationships and guide teams in residential care settings.
*Excellent communication and IT skills.
*Proven track record in managing complex care packages and supporting individuals with mental health conditions, personality disorders, autism, and learning disabilities.
