Job Overview This role is vital in supporting patient flow within the Frailty Same Day Emergency Care (FSDEC) unit. Responsibilities include same-day assessments, rehabilitation, re-enablement, and discharge planning to ensure seamless care for patients with complex needs.
Key Responsibilities *Perform same-day assessments and facilitate timely discharges within FSDEC.
*Develop and execute individualized rehabilitation and re-enablement strategies.
*Collaborate with multidisciplinary teams to optimize patient outcomes.
*Conduct effective discharge planning to ensure continuity of care.
*Maintain accurate and high-quality documentation per NHS standards.
Requirements *HCPC Registration as a Physiotherapist.
*Demonstrated experience in patient flow, rehabilitation, and discharge planning.
*Strong ability to handle a diverse and fast-paced caseload.
*Exceptional communication and organizational skills.
*Commitment to delivering excellent, patient-centered care.
