About The Role

FTC = 2 years: January 2025 – end December 2026

We are offering a rewarding and varied role to join our team as a Dementia Adviser in West Berkshire. We provide advice and guidance to people diagnosed with dementia and their carers! As a Dementia Adviser, you will have the unique and fulfilling opportunity to help individuals to maintain their independence, improving their sense of well-being, and putting them in more control of their lives.  

Community based in West Berkshire with homeworking elements
Our service caters to each individual as it is based upon their personal circumstances and support needs. The person-centred support connects with people other services, providing signposting and referrals. You will also play a role in raising the awareness of dementia in the community, with members of the public, people affected by dementia and with healthcare professionals. We also deliver awareness sessions and information programmes providing information, guidance and signposting, which you will be involved with. *Supporting people affected by dementia, families and carers to access vital services.
*Improving people’s sense of well-being, enabling them to have more control over their lives.
*Assisting with the identification of needs, providing information and access to relevant services.
*Promoting our amazing Dementia Care Advice service and building relationships with a range of health and social care professionals.
About you *Ability to supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives.
*Experience with communicating with a wide range of people, adapting your approach to ensure understanding at all levels.
*Able to network effectively and collaborating with other professionals to achieve a positive outcome for people.
*IT skills, experience working with patient or client databases and virtual meeting software (Teams / Zoom).
*Ability to travel across West Berkshire independently is essential.
