Job Overview:
Become a vital member of the Community Rapid Response Team, delivering urgent, patient-centered occupational therapy services. This role focuses on swift assessments and interventions to prevent hospital admissions and support timely discharges.
Key Responsibilities:
*Conduct urgent assessments for patients in community settings.
*Develop and implement individualized treatment plans to promote independence.
*Collaborate with the multidisciplinary team to coordinate care effectively.
*Support patients with complex needs to avoid hospital admissions.
Key Requirements:
*HCPC registration as an Occupational Therapist.
*Minimum of 6 months NHS experience in community or rapid response settings.
*Strong clinical decision-making and communication skills.
*Ability to work independently in a fast-paced environment.
