Responsibilities:
*Conducting mental health assessments to understand patients’ needs, concerns, and risks.
*Developing personalized care plans with the patient, family, and multidisciplinary teams.
*Providing immediate support to patients in mental health crises, which could include prevention of hospital admission.
*Liaising with social workers, occupational therapists, and other health professionals to coordinate comprehensive care.

Qualifications and Requirements:
*Fully enhanced valid DBS
*Mental Health Nursing degree required.
*Community experience is required.

Rate Breakdown:
£27ph LTD | £24.29ph PAYE Inclusive
