Job Title: Learning Disability Support Worker
Location: Cumbria, Carlisle
Salary: £11.50 per hour
Job Type: [Full-time/Part-time]
Overview: We are seeking a compassionate and dedicated Support Worker to provide high-quality, person-centred support to individuals. The successful candidate will work collaboratively with service users, colleagues, and external professionals to promote independence, well-being, and healthy life choices.
Key Responsibilities:
Service User Support:
* Promote the rights and responsibilities of service users.
* Work as an effective team member to ensure the best outcomes for individuals.
* Provide advice and information to service users and relevant stakeholders.
* Deliver tailored interventions in line with individual care plans using evidence-based models.
* Liaise with external professionals such as GPs and Social Services.
* Maintain accurate records and contribute to service monitoring requirements.
* Conduct risk assessments and implement effective risk management strategies.
* Ensure adherence to Health & Safety requirements.
* Assist in the facilitation of effective care pathways.
* Support individuals in achieving personal goals in accordance with their care plans.
* In residential settings, ensure service users take prescribed medication on time, providing assistance when necessary.
* Facilitate access to community-based services and resources to enhance service users’ quality of life.
Quality Assurance:
* Maintain high-quality standards and meet performance criteria.
* Participate in data collection and service improvement initiatives.
* Contribute to the continuous development of the service.
Personal Development:
* Engage in regular performance management meetings to set and review objectives.
* Participate in relevant training and professional development opportunities.
Health & Safety & Risk Management:
* Maintain a safe and clean working environment.
* Complete risk assessments where required.
* Take personal responsibility for health and safety by reporting concerns and ensuring necessary precautions are taken.
* Adhere to all health and safety policies, including serious incident and accident reporting procedures.
Requirements:
* Strong communication and teamwork skills.
* Ability to follow care plans and implement effective interventions.
* Understanding of risk management and safeguarding procedures.
* Knowledge of relevant policies and procedures within health and social care.
