Location: Leeds Community Healthcare NHS Trust
Pay Rate: £20 - £24 per hour
Pay Mechanism: Direct Engagement
Contract Duration: 28/03/2025 - 30/04/2025

Job Overview We are currently seeking a dedicated and skilled Band 6 Occupational Therapist for a community-based role at Leeds Community Healthcare. The successful candidate will work across various community settings, providing patient-centered care and promoting independence.
Key Responsibilities * Assess, plan, and deliver occupational therapy interventions for patients in the community.
* Work with multi-disciplinary teams to support patient rehabilitation and recovery.
* Develop and implement tailored care plans to meet individual needs.
* Provide advice and education to patients, families, and carers.
* Manage a caseload, ensuring timely and effective interventions.
Key Requirements * HCPC registration as an Occupational Therapist.
* Proven experience in community-based occupational therapy.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Experience in managing complex caseloads is desirable.
